    • creating an individual prevention plan;
    • 12 preventive examinations by a pediatrician;
    • 4 out-of-hours consultations in the interval between scheduled ones in case of illness;
    • 2 clinical blood tests;
    • 2 clinical urine tests;
    • 1 co-program;
    • 2 calls to a pediatrician at home within the city at a special price;
    • informational support by a pediatrician by phone.

    The term of the program is 1 year.
